Why the heck only the victim is able to report it? This is the only rule that works like that.
That might be because the victim himself can clearly prove that he was suffering from the AK bug even if he was just hit and not actually killed.

1st screenie = screenshot which shows the top right icons of AK bug? (where there is AK, it shows green player kill green player) OR the time chaser kills you, and the count down is in middle of screen?
2nd screenie = just the full list of players in the room (when you press escape)?
Sorry I'm no staff. Well, a third screen would probably not hurt: one for showing the act (hitting on the very last seconds with the name of the culprit), one for showing that at the very first seconds of the next round you get damage/are killed/stunned/pushed and the last screen showing the Esc userlist.
As for your second question: the German users already asked the same question but until someone can answer it I would say yes, it means exactly that you have to screen the full list of players in the room when you press escape.
